Receive and Use Your Kit: Follow the simple instructions included with your kit for hassle-free screening at home.
Find Your Results: Please return the kit by mail to receive your results. Returns are always free. Your detailed results will be available on the results portal, providing you with clear action steps and advice.

What we screen
Cholesterol Connect™ offers you a comprehensive cholesterol screening. This includes:
- Complete lipid panel, including LDL-C*
- Lp(a) – not typically ordered by a clinician, but an important risk factors for heart disease
Ask your medical team or a Family Heart Foundation Care Navigator about a lipid panel and Lp(a) test or learn more through the Family Heart Foundation website and the LDL Safe Zone.
- Complete lipid panel also includes HDL-C, total cholesterol, and triglyceride levels.
Why heart health screening saves lives
High low-density lipoprotein cholesterol (LDL-C) and high lipoprotein(a), or Lp(a), level (pronounced L-P-little-A), put you at risk for cardiovascular disease, especially if you have a family history.
Familial hypercholesterolemia (FH), a cause of very high LDL-C levels, and Lp(a) disorder are genetic disorders that can be screened for with blood tests and family history. Cholesterol Connect™ offers you a screening that can guide you and your family to a heart healthy future.
Lp(a) disorders occur in all races and ethnicities but have a higher prevalence in Black and South Asian individuals.
Remember, Lp(a) screening is not part of the standard cholesterol screening that your healthcare provider orders. Ask your healthcare provider about Lp(a) screening today.
